Changes of the serum and dialysate leptin in continuous ambulatory peritoneal dialysis patients with time

Abstract
Backgroud:The aim of this study was to investigate the changes of fat mass and the serum and dialysate leptin among CAPD patients with time.

Methods:52 patients were enrolled. The S- and D-leptin and BIA were measured at the 2nd week and the 12th month after initiation of PD.

Results:1) There were significant increases in fat mass, but significant decreases in the ECF% and lean body mass at the 12th month after initiation of PD (p<0.01, respectively). 2) There were significant increases in the S-leptin, D-leptin and D-VEGF at the 12th month after initiation of PD (p<0.00, respectively). 3) There were significant increases in the fat mass, S-leptin and D-leptin at the 12th month after initiation of PD in both males and females, according to the gender (p<0.05, respectively). According to the etiology, there were significant increases in the fat mass, S-leptin and D-leptin in the diabetic patients (p<0.001, respectively). 4) Factors associated with the changes of fat mass during the first 1 year were nPCR, D/P 4hr Cr, glucose absorption via the peritoneal cavity and the duration of dialysis (p<0.05, respectively). 5) Factors associated with the changes of S-leptin during the first 1 year were diabetes, % fat mass and the hs-CRP (p=0.05, respectively). Factors associated with the changes of D-leptin were D/P4 Cr, % fat mass and the S-leptin (p<0.05, respectively) 6) Factors associated with the changes of D/S leptin during the first 1 year were D/P 4hr Cr, % fat mass, hs-CRP, the duration of dialysis and the D-VEGF (p<0.05, respectively) 7) There were significant correlations between delta D-VEGF and delta D/S leptin during the 1st year.

Conclusions: Increased leptin levels of serum and dialysate might be associated with the fat mass in PD patients with time.
